Chronic Disease Management and Prevention Program
The chronic disease prevention and management program is supported through a partnership between the Division of Aging and Public Health.
Healthy Housing Program
Located in several independent living, affordable senior housing complexes in the county, this program uses public health nurses to aid residents in staying independent in their homes, rather than having to move to a nursing home. On a weekly basis the nurses visit:
- residents who are most at risk for further disability because of their chronic illnesses, and
- residents who seek a one-time consultation.
Nurses provide one-on-one education, refer residents to community resources or directly intervene with the resident’s health care provider to facilitate a change in their regular medical routine. Trained volunteers from the community support these frail residents with companionship and transportation to medical appointments and shopping. Group education programs are provided, such as “heart healthy eating” and "how to prevent falls," benefit all older adults living in the residence.
Chronic Disease Self-Management Classes
Trained professionals and volunteers teach a six-week workshop on how to improve your health while living with a chronic disease. Class fees are $10 for the series and include a workbook designed by the Stanford Chronic Disease Self-Management Program. Call 415-499-6376 to find out where the next class is and to register.
