Birth Certificates:
Birth records
are maintained at the Marin County Office of Vital Statistics for the
current year and the previous year only for all Marin County births
(birth certificates those born outside of Marin County must be obtained at the
County or State office of birth). All other past years including the years mentioned
above are kept at the County Recorder’s office in the Civic Center building

Birth Certificates can be requested either in person or by mail.
Birth Certificate orders by mail:
- Application forms are provided to the mother prior to discharge from the hospital with instructions on how to obtain the birth certificate. You may request that an application form be sent to you through the US Mail by calling (415) 499-6876, or you can download and print as follows:
- Application must be completed and mailed in with check or money order in the amount of $14.00 (please call if you need assistance in completing the application, 415-499-6876).
- Certificate of Acknowledgement must be completed and signed in presence of a Notary Public. If this is not done requestor must come into the office for in-person signature.
- If ordering multiple birth certificates, separate “Application for Certified Copy of Birth Record” must be submitted.
- Multiple birth certificates may be ordered w/ one Certificate of Acknowledgement however the sworn statement must include the name of each individual whose birth certificate is being requested with the person’s relationship to that individual. The $14 fee applies to each certificate being ordered.

- The birth certificate will be mailed to you at the address indicated on the “Applicant Information” section of the application form within 2 business days of receipt of application and/or registration of birth.
* Please Note: birth record must be received by Marin County Office of Vital Statistics from the hospital of birth before a birth can be registered and birth certificate produced. If receipt of the birth certificate is time-sensitive, please call the Marin County Office of Vital Statistics at 415-499-6876 to verify that record has been received from hospital.
Birth Certificate orders in-person:
- Birth Certificates may be obtained on a drop-in basis in the Marin County Office of Vital Statistics
- Application forms are available at the office or can be printed out and completed prior to arriving at the office. You must sign the Certificate of Acknowledgement section in the presence of Vital Statistics staff.
- The fee is $14.00 for each birth certificate being requested payable by check, money order, or cash.
Death Certificates:
Death records are maintained at the Marin County Office of Vital Statistics for the current year and the previous year only for all Marin County deaths (death certificates for deaths that occurred outside of Marin County must be obtained at the County or State office of place of death). The Medical Marijuana ID Program, provided by the Marin County Department of Health and Human Services, Office of Vital Statistics, offers seriously ill Marin County residents an efficient and confidential method to verify to law enforcement representatives that they qualify for the use of medical marijuana. This program is consistent with the intent of Proposition 215 (California Health and Safety Code-Section11362.5). This service is available to both patients and their primary caregiver. The current fee for the State of California Medical Marijuana Photo ID Card is $113 (Medi-Cal patients receive a 50% reduction in this fee). The following information outlines the policies and procedures that will be used to issue cards.
PROCEDURES FOR PATIENTS
- Patients
wishing to be certified for the Medical Marijuana ID Card Program may call
the Marin
County Office of Vital Statistics
at 415-499-3288 to request an application packet. Application packets can
be obtained in person at 20 N. San Pedro Road/Room 2006, San Rafael, CA (office
hours are Monday-Friday 8:30am-4:00pm). An application packet may also be
downloaded and printed via the links above. Application packets include: information
about SB 420 and the County ID Card Program, Frequently Asked Questions and,
the Medical Marijuana Photo ID Card Application.
- Applications
are processed BY APPOINTMENT ONLY at the Marin
County Office of Vital Statistics.
Call (415) 499-3288 to make an appointment. Marin
County Office of Vital Statistics
personnel are required by the State of California to verify the following
information at the appointment:
- The application is
complete, including the physician's section.
- The physician named
by the applicant currently holds a license by the State of California.
- The physician has
orally confirmed the recommendation of the use of medical marijuana for
the patient.
- Patients' proof of
identity in the form of a photo ID: California State driver's license
or ID card; United States Passport; or Veteran's Administration ID Card.
- Patients' proof
of residency in Marin County in the form of a California State
driver's license; a County ID card with a local address; or a recent utility
bill, recent mortgage or rental payment that includes the applicants address.
- If applicable, the
patients' proof of Medi-Cal status (there is a reduced fee for the Medical
Marijuana Card for Medi-Cal patients).
- After
verifying the information above (by appointment only), the applicant will
be approved for the Medical Marijuana ID Card Program. A fee will be collected
and the applicant's photo will be taken (State Card Fee = $113.00, State Card
Medi-Cal fee is $56.50). The photo, Medi-Cal status (if applicable), and Medical
Marijuana ID card expiration date will be sent to the State of California
via a secure web site. The State of California does not receive any other
identifying information, such as name or address, from the Office of Vital
Statistics.
- The
State of California will issue a photo ID card with the patient's photo, date
of expiration, ID number, card verification internet address, and local Office
of Vital Statistics contact information. The cards will be sent to the Marin
County Office of Vital Statistics
within 7 days.
- ID cards can be picked up at the Marin County Office of Vital Statistics upon receipt from the State of California. Cards will be ready 7-10 days after processing and you will be given the pick up date at your appointment.
- The fee for Medical Marijuana ID cards is $113. The Medi-Cal fee is $56.50. Cards will be valid for one year from date of application. Applicants must contact the Marin County Office of Vital Statistics to renew their certification (415-499-3288). No notice of expiration will be issued.
- Lost or stolen ID Cards must be reported immediately so that they can be invalidated and a new card issued. Applicants should keep a record of their ID number in a safe place for reference. MEDICAL MARIJUANA ID CARD PROGRAM:
PROCEDURES FOR PRIMARY CAREGIVERS
- For the purposes of the Medical Marijuana ID Card Program, a primary care giver is an individual "who has consistently assumed responsibility for the housing, health, or safety of a patient who is applying for certification with the program. In order to be certified as a Primary Caregiver for the Medical Marijuana ID Card Program, a patient who is approved for the program must identify the Primary Caregiver as such in section 4 of their application.
- Applications

at 499-3288. Applicant/caregiver may pick up packets from the Marin
County Office of Vital Statistics,
20 N. San Pedro Road/Room 2006, San Rafael, CA., Monday-Friday 8:30am-4:00pm.
